Caryn Dinetz, 49, was captured at her residence in the Candlewood Suites hotel in Nanuet, N.Y., and was charged with identity theft, grand larceny, unauthorized practice of a profession and scheme to defraud, Manhattan District Attorney Robert Morgenthau said.

Dinetz stole Karen Levine’s identity by lifting her licensing information from the Web site of the state Office of Professions, which licenses physical therapists, and then posing as her from March 1, 2005, to Feb. 16, 2006, Morgenthau said.

Dinetz, who is not a licensed physical therapist, had Levine’s documents reissued to her after telling officials they had misspelled her first name, that Levine was her maiden name and that she wanted “Dinetz” added to her license, Morgenthau said.

She found jobs by submitting resumes in the name of Caryn Levine Dinetz and presenting the required medical malpractice insurance forms using Levine’s policy number directly to health centers, prosecutors said.

While using Levine’s identity, Dinetz worked at the Ezras Choilim Health Center in Orange County. The defendant, a Florida native, is a fugitive from credit card fraud charges in that state, prosecutors said.
